Coming to the office during the COVID-19 pandemic Rules to be followed prior and during your visit

 

Dear Patient:

You were given an appointment in our office for a consultation, and will be seen shortly.While the rules regarding movements have been relaxed, there is still a need for stricts rules to minimize your risk of contracting the disease as well as the risk of transmitting COVID-19 to other people. This virus can be transmitted easily to others when you are symptomatic, but also if you are not.

To date, only a very small percentage of the population has contracted COVID-19 and has developed an immunity to the virus. Hence, the risk of spread remains high.

Many of our patients are elderly, many have more than one risk factors.We have taken measure to minimize the risk of spread but these measures will only be effective if YOU COLLABORATE. For the sake of our staff, your own safety and that of our patients at risk, we ask you to follow the following recommendations.

Please abide by the following rules:

- Come to your appointment at the specified time. This will limit the number of patients in the waiting room to a minimum. .

- Only essential people accompanying a patient will be allowed in.They are subject to the same rules (and screening requirements) as the patient.Whenever possible, accompanying people and family should wait outside the office.They will be called back prior to the end of the consultation.

- For children, only one parent should accompany the child. No other sibling should be present, unless he/she is also scheduled for an exam.

- Whenever possible, bring your own mask. It will be needed during the exam. If you do not have one, we will provide you one. However, our stock is limited. Please ask when you arrive at the office.

- Respect recommended safe distances of 2 meters whenever possible.This applies also while you are seated in the waiting room

- A hand lotion dispenser is provided as you enter the office. Please make use of it upon your arrival, and at anytime you touch your face or mouth.

- On the day PRIOR to your office visit, please fill the questionnaire at the end of this letter. If you asnwer “YES” to any of the questions in 2 to 4, contact the office by phone (021-566-1222). If there is no one answering, leave a message so we can call you back.

- If you answered “YES” to any of the questions in sections 2 to 4, you must not come to the office before we give you specific instructions.Your appointment may be moved to a different time slot or day.

- Please bring the filled questionnaire with you to the office.
